This is the most insidious pattern. The POC works. The demo is compelling. Everyone agrees it should go to production. And then... nothing. The pilot lives in a sandbox forever because nobody planned for integration, change management, or the operational disruption of deploying a new decision-making system into a live workflow. The pilot becomes a permanent science project.

4. Measuring the Wrong Things

AI teams report model accuracy. Operations teams care about first-time fix rates, average handle time, and SLA compliance. When the metrics don't align, the AI team celebrates a 94% accuracy rate while the field team sees no improvement in their daily reality. Define success in operational terms from the start — not in model performance terms.

5. Underestimating Change Management

A dispatcher who has been routing technicians for 20 years isn't going to trust an algorithm overnight — and shouldn't have to. The most successful AI deployments I've seen invest as much in training, communication, and gradual rollout as they do in the technology itself. Trust is earned through transparent, explainable, incremental improvement.
